BOY SCOUTING
Boy Scouting, one of the traditional membership divisions of the BSA, is
available to boys who have earned the Arrow of Light Award and are at least 10
years old or have completed the fifth grade and are at least 10, or who are 11,
but not yet 18 years old. The program achieves the BSA's objectives of
developing character, citizenship, and personal fitness.

Annual
Health and Medical Record - Parts A&B
To be completed
at least annually by participants in all Scouting events. This health
history, parental/guardian informed consent and hold harmless/release
agreement, and talent release statement is to be completed by the
participant and parents/guardians. |

Annual Health and Medical Record - Part C
The physical exam
that is required for participants in any event that exceeds 72 consecutive
hours, for all high-adventure base participants, or when the nature of the
activity is strenuous and demanding. Service projects or work weekends may
fit this description. Part C is to be completed and signed by a certified
and licensed heath-care provider—physician (MD or DO), nurse practitioner,
or physician assistant. It is important to note that the height/weight
limits must be strictly adhered to when the event will take the unit more
than 30 minutes away from an emergency vehicle, accessible roadway, or when
the program requires it, such as backpacking trips, high-adventure
activities, and conservation projects in remote areas.

Annual Health and Medical Record - Part D
Required to be
reviewed by all participants of a high-adventure program at one of the
national high-adventure bases and shared with the examining health-care
provider before completing Part C.

Medication Form

Troop Documents
Scouts and
Parents: All scouts are required to submit a Medication Form in a ziploc bag
with the scouts medicine to the Scoutmaster (SM) before any campout or day
activities where medication is required. If the SM is not available,
please provide the form and medicine to the Assistant Scoutmaster (ASM) in
charge of the activity. Scoutmasters are not allowed to administer
medication without a signed medication form. Only the medicine
detailed on the form can be administered by a SM or ASM. Also, any
scout that has been sick or under medical care during the week before camp
must notify the SM or ASM in charge about their condition and what
activities they are limited to. Please help us with this process so we
can assure your safety at camp or activities.
